General Incident Report

General Incident Report: Use this form to create a report about any alleged policy violations at PAU, including both academic and community-based policy violations. With the exception of reports of sexual misconduct and gender-based discrimination (Title IX), and Identity-Based bias incidents, all reports of an alleged violation of University policy should be reported to the Department of Student Success through the General Incident Report.
